The reason such a change in the name is due to Petitioner requesting the restoration of her maiden name post-divorce proceeding. Petitioner desires to have the Hart last name, to-wit: Amanda Joy Hart. 5. This petition has not been submitted with the intention of defrauding another of any rights.
